Tool 5.1 Sample Workflow Diagrams

This workflow diagram shows a simplified sequence of steps related to writing a prescription by a physician. It's made to appear hand-drawn to emphasize the possibility of creating the diagram by hand rather than using a computer tool. The diagram begins with the patient arriving and ends with the patient leaving. Process steps are shown as rectangles, decision steps as diamonds, and beginning and end points as ovals with text inside them. Unless specified, all flowchart symbols are process steps (i.e., rectangles). The texts inside the flowchart symbols are shown with quotation marks.

The individual steps shown are as follows: 
The process starts with "Patient arrives"(oval), and connects to "Patient checks in," which then connects to "Dr. Exam," meaning that the doctor examines the patient. 
Another workflow starts in parallel with "Nurse checks schedule," expecting patient to arrive, connects to "Pull chart," which then connects to "Dr. Exam," joining the step described earlier. 
Following "Dr. Exam" is a decision point of "Prescription needed?" (diamond). If "No," it connects to the end point "Patient leaves" (oval). If "Yes," the following step is "Write paper prescription." 
The prescription writing step connects with two subsequent steps. One step is "Copy to patient," meaning giving patient a copy of the prescription. It then connects to the end point "Patient leaves" (oval). 
The other step is "Copy to chart," meaning keeping a copy in patient chart. It then connects to "Return chart," meaning filling the chart back to file storage.

Figure 5.3a in the toolset document. 

This diagram shows a sequence of steps concerning prescribing in a provider's office. It starts with the patient arriving at the provider's office. The middle is a large rectangular box encompassing 6 steps in the provider's office related to prescribing. It ends with the patient taking a prescription to a pharmacy. Process steps are shown as rectangles, steps for handling or producing documents as rectangles with a curved (like paper) bottom, and beginning and end points as ovals with text inside them. Unless specified, all flowchart symbols are process steps (i.e., rectangles). The texts inside the flowchart symbols are shown with quotation marks.

The individual steps are as follows: 
The process starts with "Patient visit provider"(oval), and connects to "Prescriber assesses patient," which is a step inside the big rectangle of "Provider's Office." 
In addition to "Prescriber assesses patient," the other five steps inside "Provider's Office" are two process steps "In-office prescribing of meds" and "Physician adds note to chart and returns chart to staff" and three document steps (in rectangles with wavy bottom) "Staff retrieves patient chart," "Staff returns patient chart to file storage," and "Patient receives one or more prescriptions." 
The step following "Prescriber assesses patient" is "In-office prescribing of meds." Its other preceding step is "Staff retrieves patient chart." 
"In-office prescribing of meds" is connected to two steps. One is a process step "Physician adds note to chart and returns chart to staff," followed by a document step "Staff returns patient chart to file storage." The other is the step "Patient receives one or more prescriptions." This latter step connects to the final step, "Patient takes prescriptions to pharmacy" (oval), outside "Provider's Office."

Figure 5.3b in the toolset document. 

This diagram shows a sequence of steps starting with the patient needing a renewal of a prescription, and deciding to contact provider or a pharmacy. Tasks occur within two boxes, one set at the physician's office and another set at a pharmacy. It ends with the patient picking up a prescription at the pharmacy. Process steps are shown as rectangles, decision steps as diamonds, steps for handling or producing documents as rectangles with wavy bottom, and beginning and end points as ovals with text inside them. Unless specified, all flowchart symbols are process steps (i.e., rectangles).

The individual steps shown are as follows: 
The process starts with "Patient needs a renewal"(oval), and connects to a decision point "Patient contacts Provider or Pharmacy"(diamond), which then branching out to either "Provider's Office" on the right side of the figure encompassing 8 steps or "Pharmacy" on the left side of the figure consisted of three steps. 
If patient decides to contact the pharmacy in the decision point, it follows with the step "Pharmacy determines need for renewal," which then connects across the two large rectangle boxes to the first step inside "Provider's Office" via "Phone or Fax." 
The first step in "Provider's Office" is "Clinic staff transcribes or retrieves request." 
The second step is "Staff retrieves patient chart." 
The third step is "Staff deposits message, fax and charts for prescriber approval." 
The fourth step is "Prescriber retrieves documents & approves, changes or denies renewal or exception." 
The fifth step is "Prescriber passes chart and prescriptions to staff." This step branches out to two subsequent steps. To the right side is "Staff files chart," which connects upward to a document step "Patient chart files"(rectangles with wavy bottom) meaning filing patient chart in file storage, following by a connection back to the second step. 
To the left side of the fifth step is "Renewal approval is transmitted to pharmacy," which then connects to "Pharmacy receives approval," a step in "Pharmacy" large box. It is followed by "Pharmacy dispenses meds." 
Next and final, the sequence ends with "Patient picks up prescription"(oval). 

Figure 5.4a in the toolset document. 

This diagram shows a sequence of steps of in-office prescribing with a electronic prescribing system. It starts with the patient arriving at the provider's office. It ends with the patient picking up medication at pharmacy. The middle is a large rectangle box consisting of process steps occurring at the provider's office involving electronic prescribing. The process steps are shown as rectangles and beginning and end points as ovals with text inside them. All flowchart symbols are process steps (i.e., rectangles) unless specified. The texts inside the flowchart symbols are shown with quotation marks.

The individual steps show are as follows: 
The process starts with "Patient visit provider"(oval), and connects to "Prescriber assesses patient," which is a step inside the big rectangle of "Provider's Office."
