Summary:
- HIT Description: Computerized decision support system; Computerized provider order entry More info...
- Purpose of Study: Evaluate the impact of computerized ordering guidelines on clinician test ordering for neuro-radiology imaging studies of the head.
- Years of study: 2000-2001
- Study Design: Pre-post.
- Outcomes: Utilization and questioned costs.
- Settings: An academic university medical center with 630 beds and approx 31,000 admissions per year.
- Intervention: A computerized decision support system integrated into a computerized clinician order entry system compared to care without the computerized decision support system.
- Evaluation Method: Utilization of services and question costs.
- Description: A locally developed clinical practice guideline was developed for the appropriate imaging study for specific clinical conditions. The decision support system was triggered when an order was placed for a neuro-imaging study. This was incorporated into a computerized clinician order entry system called WizOrder.
- Healthcare Utilization: There were 742 total orders placed during the pre-intervention phase and 704 orders placed using the decision support system. In the intervention period there was a significant change in the distribution of tests ordered with non-contrast brain MRI studies increasing and all other types of neuro-radiology studies decreasing. In 40% of cases users who received a recommendation chose to order a non-recommended study.
