Improving Rural Health Care: Implementing Innovative Integration Solutions

Description

Integrated existing health IT technology software and systems among a three-hospital consortium to create a shared electronic record that ultimately gave providers access to real-time data and the ability to electronically prescribe medications. The goals of the project were to reduce medical errors, improve the quality of patient care, increase patient satisfaction and lower costs.

Sharing Patient Record Access in Rural Health Settings

Description

Developed an implementation plan for an ambulatory EMR in a medically underserved region that electronically connects physician offices, the regional hospital, ancillary services, and other community health services. The project work identified indicators that track measurable improvements in patient safety, quality of care, clinician and patient satisfaction, and operational efficiency.
