Track 8-III New Approaches in Medication Management and Care Transition
The purpose of this session is to identify strategies for improving medication safety across different settings and populations. Participants will learn how to recognize some of the causal factors linked to medication errors and develop solutions to improve medication safety. Tools discussed include a Patient Safety Council, a medication reconciliation form, and a home health care electronic order set.

Track 8-V Patient Centered Health IT: Perceptions and Approaches
This session will explore patient's perceptions and concerns related to HIE, and present various IT tools that can be used to enhance patient-provider communication. Issues discussed in this session include patient web portals, creating a culture of safety, and using HIT to reduce tobacco use.
