A National Web Conference on Using Health IT to Improve Care Coordination and Outcomes in Patients with Complex Healthcare Needs

Overall Purpose:

The Agency for Healthcare Research and Quality (AHRQ) has identified a gap in health care and public health practitioners’ knowledge of health IT using multiple mechanisms, including the findings of the continuing education (CE) planning group. This series of Webinars is designed to increase practitioners’ ability to improve health care decisionmaking, support patient-centered care, and improve the quality and safety of care through the use of health IT. A major component of providing safer and better quality care is to discover new ways to improve the ability for providers to better support individuals with complex health care needs. This presentation outlines the findings from recent research that advances knowledge about how health IT can impact outcomes for patients with multiple chronic conditions seen across a variety of settings. Included will be a review of the following: (1) coordination of care outcomes for patients with two or more chronic conditions who were discharged from a hospital to a rural primary care clinic; (2) the development of a medication management system to improve clinician practice and enhance patient engagement by identifying patients with complex medication regimens; and (3) facilitating the availability of information following care transitions including hospital and emergency department discharge and specialty care referrals into ambulatory care.

Learning Objectives:

  1. Describe the impact on patient outcomes and hospital readmissions of an EHR-generated “discharge checklist” used to facilitate information sharing with primary care providers for patients with multiple conditions.
  2. Discuss the factors associated with the use of a clinical decision support tool to reduce risk of adverse events due to complex medication regimes in a home health setting.
  3. Identify the lessons learned of implementing a health information exchange-based clinical decision support system triggered during transitions in care for patients with complex needs.
