Description

Developed, implemented, and evaluated a cooperative effort for using health IT to facilitate a continuum of appropriate medical and developmental care, from the time infants are admitted to Neonatal Intensive Care Units (NICUs), through the transition process to community-based health care services for infants most at-risk for long-term neurodevelopmental problems.

Integrated an office-based EMR within an acute care hospital, rural community health centers, a community mental health center, a family medicine residency, private physician practices, and a home nursing service for improving use of the EMR as a clinical tool, integrate clinical data, and increase access to the data.

Bringing Measurement to the Point of Care

Description

This project deployed new functionality that allowed clinicians to view ‘real time’ measures of the delivery of clinical preventive services and added patient specific alerts and ‘one click’ options to act on missed opportunities for followup care.
