This project involved the development of an implementation plan for an ambulatory electronic medical record in a medically underserved region of east-central Illinois. Sarah Bush Lincoln Health Center (SBL), the fiscal agent, partnered with independent physician practices to develop an implementation plan for an ambulatory electronic medical record (EMR) provides shared access to patient records across hospital services, home health, hospice, and physician practices. The EMR will electronically connect physician offices, the SBL regional hospital and its ancillary services, and other community health services. We intend to develop and share a longitudinal medical record that contains a patient problem list, medication list, allergies, radiology images and data, laboratory data, and a patient care plan. The plan was researched through participation in meetings and conferences calls, literature reviews, meeting with software vendors, and visiting hospitals and clinic sites. As a result of the planning grant period, SBL developed a project plan that served as a guide to implementation of the above applications. Additionally, the planning grant period prepared SBL for the submission of their implementation grant application.
