Description:Many current and proposed projects present a specific type of information or information from a specific source to care providers using a pre-determined content or format.  This can be a barrier to the full use of the resource if providers think the presentation is inefficient or inappropriate.  The goal of the proposed research is to develop, implement, and evaluate a prototype for a scalable and widely deployable knowledge delivery service ("knowledge broker") capable of automatically delivering context-specific knowledge from online resources into electronic health record (EHR) systems via a "knowledge dashboard." This will help providers address their knowledge gaps, leading to better and more informed decisions.

Description:Memorial Hospital of Rhode Island has designed a two-phase study of the feasibility and acceptability of an e-health model for the treatment of hypertension.  The study, the eHealth Blood Pressure (BP) Control Program, integrates electronic medical records (EMR) and personal health records (PHR) with monitoring devices through a Web portal connecting patients to their medical team.  With patient education, collaborative self-management support, and care coordination, the program strives to increase medication adherence, reduce clinical inertia, and improve patients' blood pressure control.  This will provide the pilot data needed for a randomized practical clinical trial of the eHealth BP Control Program.
