Impact of a Wellness Portal on the Delivery of Patient-Centered Prospective Care

Description:

The goal of this project is to bridge the gaps in current knowledge base and identify a health information technology (HIT) strategy that will promote patient-centered care via the development and testing of an Internet-based patient Wellness Portal. Patient-centeredness is particularly important in primary care as most individuals seek care in this setting. The use of HIT is essential in integrating patient-centeredness into medical practice. A coordinated and patient-centered delivery system that is reinforced with an integrated implementation of HIT can substantially improve the safety and quality of care in primary care practices.

Privacy and Security Solutions for Interoperable Health Information Exchange / Oklahoma
Description:

Thirty-three states and 1 territory formed the HISPC, which aims to address the privacy and security challenges presented by electronic health information exchange through multi-state collaboration. With the support of its state or territorial governor, each team engaged a steering committee and a range of local stakeholders to 1) assess variations in organization-level business policies and state laws that affect health information exchange; 2) identify and propose practical solutions, while preserving the privacy and security requirements in applicable Federal and State laws and 3) develop detailed plans to implement solutions.

Implementation of Health Improvement Collaboration in Cherokee County, OK

Description:Implemented a HIPAA-compliant electronic records system to improve public health, patient safety and facilitate information sharing among health care providers in the Talequah area, with the goals of establishing a Web-based information and referral service and creating a community-wide, science-based prevention strategy supported by the new information system.

Health Improvement Collaboration in Cherokee County, Oklahoma
Description:Created a plan for developing an integrated, multifunctional, HIPAA-compliant Community Health Information Network; developed a telephonic comprehensive nurse line service and triage function; and investiged and implemented improvements for streamlining of existing appointment systems.

Delivery of Preventive Services in Primary Care
Description:Disseminated strategies targeted toward increasing immunizations and preventive care, including a computer reminder system to practices in a Practice-based Research Network (PBRN). Assessesed, using a randomized controlled trial methodology, the relative adoption success of academic detailing combined with and without practice facilitation and technology implementation assistance, on the delivery of preventive services.
