| Description: The primary goal of this proposal is to conduct a randomized clinical trial to evaluate the effectiveness of an integrated electronic health record system that includes a eMedical Record (EMR), eDental Record (EDR), and a Personal eHealth Record (PHR) to improve the quality and safety of dental care for patients with chronic illnesses including diabetes, heart disease, pulmonary disease, and conditions that cause xerostomia (dry mouth). If these medical conditions are not identified and addressed in routine dental care, the quality and safety of the care is adversely affected. |

| Description: Achieve Healthcare Information Technology, MN, in conjunction with nursing facilities in Minnesota associated with the Benedictine Health System and Preferred Choice Pharmacy will be the first program to evaluate how the e-prescribing standards work in certain long-term care settings. The project also will assess the impact of e-prescribing on the workflow among prescribers, nurses, the pharmacies, and payers. |

Privacy and Security Solutions for Interoperable Health Information Exchange / Minnesota

| Description:Thirty-three states and 1 territory formed the HISPC, which aims to address the privacy and security challenges presented by electronic health information exchange through multi-state collaboration. With the support of its state or territorial governor, each team engaged a steering committee and a range of local stakeholders to 1) assess variations in organization-level business policies and state laws that affect health information exchange; 2) identify and propose practical solutions, while preserving the privacy and security requirements in applicable Federal and State laws and 3) develop detailed plans to implement solutions. |
